I think can download the form from http://www.scheme6000.gov.hk/chi/forms.html

and drop to the box at post office (if want cheque) or bank (if want autopay)

just take a form from any retail banks and fill in the personal data and then send it back to the bank (if he wants to deposit in HSB, you need to send the form back to the destinated bank which is HSB in this case).
